OVERVIEW

Our core service offering individualized support and wraparound services, pairing young people with a Youth Navigator and assessing needs, developing a plan and connecting youth with other Project Legacy programs and services.

PROGRAM COMPONENTS

The following elements are involved in Support 365:

  • Educational and career planning

  • Meals and groceries

  • Mental health support

  • Physical health support (health insurance, dental, vision, etc.)

LOCATIONS

Support 365 services are available to youth in the following locations:

  • Rochester, Minnesota

  • St. Paul, Minnesota

  • Ely, Minnesota
